Understanding Medicare Coverage for Physical Therapy, Occupational Therapy and Speech Therapy

What Medicare Part B Covers

Medicare Part B (Medical Insurance) covers outpatient physical therapy services (includes in-home or house calls) when they are:

  • Medically necessary
  • Provided by a Medicare-certified physical therapist
  • Reasonable and appropriate for your condition
Covered Physical Therapy Services Include:
  • Initial evaluation and assessment
  • Treatment sessions with a licensed physical therapist
  • Therapeutic exercises and manual therapy
  • Gait and balance training
  • Pain management techniques
  • Home exercise program instruction
  • Progress evaluations

Do You Need a Referral?

No Referral is required. New Jersey has direct access laws allowing you to see a physical therapist without a doctor's referral, howevwr, Medicare does require that your care be under a physician's plan of care. Here's what you need:

  1. Initial Doctor Visit: You will need a medical doctor/ primary care physician following up on your medical condition.
  2. Plan of Care: Your physical therapist develops a treatment plan that your medical doctor reviews and approves
  3. Recertification: Your doctor must recertify your plan every 90 days if therapy continues

At Holsman Physical Therapy, we coordinate closely with your physician to ensure all Medicare requirements are met.

Common Conditions We Treat in Seniors

Fall Prevention and Balance Issues

Falls are the leading cause of injury among seniors, but they're largely preventable. Our evidence-based No-Fall Protocol™ has helped hundreds of seniors improve balance and reduce fall risk.

Balance and Fall Risk Assessment Includes:

  • Balance testing (Berg Balance Scale, Timed Up and Go test)
  • Gait analysis
  • Strength evaluation
  • Home safety assessment
  • Vision and medication review (coordinated with your doctor)

Fall Prevention Program Components:

  • Balance training exercises
  • Strengthening for leg and core muscles
  • Gait training with our HolStep™ system
  • Coordination and reaction time exercises
  • Education on fall prevention strategies
Results: Studies show that comprehensive fall prevention programs can reduce fall risk by up to 40%.

Stroke Recovery and Rehabilitation

Stroke is a leading cause of long-term disability, but intensive rehabilitation can dramatically improve recovery. Physical therapy helps stroke survivors:

  • Regain movement in affected limbs
  • Relearn walking and functional movements
  • Improve balance and coordination
  • Increase independence in daily activities
  • Prevent complications like muscle contractures

Timeline: The first 3-6 months after stroke are crucial for recovery. Starting physical therapy as soon as medically cleared leads to the best outcomes.

Arthritis Management

Osteoarthritis affects millions of seniors, causing joint pain, stiffness, and reduced mobility. Physical therapy is a first-line treatment recommended by the American College of Rheumatology.

Common Arthritis Locations We Treat:

  • Knee osteoarthritis
  • Hip arthritis
  • Spine (cervical and lumbar)
  • Shoulder arthritis
  • Hand and wrist arthritis

Benefits: Research shows that physical therapy can reduce arthritis pain by 40% and improve function significantly—often allowing patients to delay or avoid joint replacement surgery.

Post-Surgical Rehabilitation

Physical therapy is essential after many surgeries common in seniors:

Joint Replacement Recovery:

  • Total knee replacement
  • Total hip replacement
  • Shoulder replacement

Other Surgical Procedures:

  • Spinal surgery (laminectomy, fusion)
  • Fracture repairs (hip, wrist, shoulder)
  • Rotator cuff repair
  • Cardiac surgery rehabilitation
